So far as I know, every PC keyboard has a caps lock, but not every keyboard has an insert. Given that fact, why is the default for *most* screen-readers still insert in 2026?
Not a hot-take, genuine question. Failing that, why not start with both active and you just opt out of what you *don't* want?

@FreakyFwoof If you are on desktop keyboard, at least for me it's still more convenient to use NumPad Insert than CapsLock.

@menelion Even though I have an insert key, I have never, ever used it for a screen-reader modifier. I actively avoid using the numpad for anything, so I don't get bad habits when I switch to using my laptop. I in fact use NVDA in laptop layout on purpose, everywhere.

@FreakyFwoof My right hand, when I read or navigate (not when typing), has a rest position so that my fifth finger is on the NumPad insert (zero) and my third finger is on the UpArrow key. It's so for more than two decades now.

@menelion I just pinky finger the caps lock, but I'm a Mac user that actively chooses *not* to use Caps on Mac, so I'm used to being a contortionist. I'm fine with odd finger combinations.
